Claude Sonnet 4.6 with tool-use output produces a Subjective / Objective / Assessment / Plan note as structured JSON — never freeform text. Prompt caching keeps the per-note cost down.
On sign, a second Claude pass extracts what's new — vitals, diagnoses, meds, allergies, referrals, imaging orders — and merges into the patient's longitudinal timeline. No copy-paste between systems.
Signing computes a SHA-256 hash of the note and writes a tamper-evident JSON snapshot to encrypted Cloud Storage. Edits after signing go through addenda, not overwrites.
Search any ICD-10 code on a patient's chart and Claude synthesises matched findings across history, reports and uploaded documents into one summary. No retraining — works against whatever's been signed.
